Letter To Patients
Dear Eye Center Of The Rockies Patients,
First, I want to thank you all for the kind comments, cards, letters, hugs, candy, etc. that you’ve given me as we proceed through this transition to my retirement. I feel the same way for all of you. It’s been my honor to care for you. This website will remain active as a resource and point of contact through our designated records custodian for the duration of our record retention period — up to 21 years, and no less than 10 years.
As I mentioned in my letter, I am writing to you about how to obtain copies of your medical records. We have arranged for Morgan Records Management to be our custodian for your records for the next 10 years, and longer than that for those under the age of 18. Patient records are all stored as computer files and are much easier to transfer than in the old days of paper charts. Those of you that initially had paper charts long ago have that information scanned and stored in your digital file.
I truly will miss you all and hope to see you around town.

Notes For Patients
Your health records from this Practice will be transferred to Morgan Records Management, LLC (“Records Custodian”).
The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be destroyed no less than seven years after the last date of services you received from this Practice, although most records will be held for at least 10 years from the date of last service, depending upon applicable state and federal record retention requirements.
